Dr Phil Debenham was the chair on Hayley Fullerton’s serious untoward investigation. He had no direct involvement in Hayley’s care. He was in no way responsible for her death. I would describe him as a doctor with no ego and a great listener.
He looked at the root causes into Hayley’s death. A Root Cause Analysis is an investigation which decides;
- What happened
- Why did it happen
- What do we do to stop it happening again
Unfortunately, Dr Debenham was lied to by certain staff, “stonewalled” by others and didn’t have access to certain staff. He tried his best but the report is full of lies, important facts were covered up and serious safety concerns were not even investigated.
